The Hub offers exam proctoring for in-person exams from 8:00 am – 7:00 pm Tuesday, December 10th through Tuesday, December 17th excluding Sunday, December 15th. Students begin the process by completing the survey form below. Upon submission by the student, the faculty will receive an email to complete the guidelines associated with the student’s request. The submission by the faculty completes the form. If the Hub CANNOT proctor the request the student and faculty will be contacted. All parties will receive an email with the necessary information from this request form.
All requests must be submitted by Tuesday, December 3rd at 5:00 pm.
Final Exam Exam Proctor Request Form (For Students)
If the student is taking the exam with class/proctored by faculty, the instructor and student will determine the process for the student to access accommodations
If the student is requesting exam proctoring at the Hub, the student is responsible for submitting a Hub Exam Proctor Request Form at least 3 business days in advance of the scheduled exam
If the request is submitted less than 3 business days prior to the requested exam proctoring date, the student will receive an email stating that the Hub is unable to facilitate the exam proctor request
Once the student and faculty complete their respective portions of the form, an email containing the Exam Date, Start and End Time, and Equipment Permitted will be forwarded to the student and faculty. If the Hub is unable to proctor this exam, the student and instructor will be contacted directly
Review the Letters of Accommodation for students enrolled in classes. Letters of Accommodation are provided directly to faculty via a shared Google Drive folder
Discuss with eligible students how accommodations pertain to class context and determine how to administer exams. Accessibility Services is available to consult
Review and complete the faculty portion of the “Hub Exam Proctor Request Form.” The form will be sent each time a student submits a request
Upon completion of the faculty portion of the request form, an email containing the faculty guidelines is forwarded to the Hub and the faculty. As noted above an email is forwarded to the student containing only the Exam Date, Start and End Time, and Equipment Permitted. If the Hub is unable to proctor this exam, the student and faculty will be contacted directly
Faculty are asked to deliver a hard copy of the assessment (quiz/exam) and all required materials to the Hub/3rd Floor of Scott Hall
To maintain the integrity of the format and content, a hard copy of the exam should be delivered by 9:30 AM the day prior to the date the exam is to be taken. If the student is taking the exam on Monday, Friday drop off is requested
All completed exams will be secured at the Hub. Faculty can pick up exams or arrange for pick up
