Documentation for ADHD accommodations may include (but are not limited to):

  • A comprehensive, psychoeducational or neuropsychological assessment within the past three years that is scaled for adults (report should include standard scores, standard deviations and percentiles)
  • Evidence of current impairment
  • A specific diagnosis per the DSM-V
  • Information on side effects of medications if relevant to an accommodation request
  • Summary that addresses any functional limitations the student’s disability has on the academic setting
  • Recommended accommodations

Since the onset of ADHD occurs in childhood, there should be evidence of the impairment beginning in childhood. Additionally, since ADHD evolves as individuals become older documentation should be fairly recent and preferably within the last three years. If the documentation is older than this, then the specialist whom the individual has history with should submit an update summary.
